How to Receive Your Alcon Contact Lens Rebate:
Get an eye exam and/or contact lens fitting.
Purchase the required boxes of contact lens products as indicated between July 1, 2015 and January 31, 2016.
Go to www.myalconlensrebates .com and enter your Individual Rebate Code.
Print your Alcon Official Rebate Form, attach the following required documents, and mail in within 60 days of your contact lens purchase date (You’ll receive the mailing address when you complete the rebate registration process at www.myalconlensrebates .com).
a) Original sales receipt with Alcon product(s) circled
b) One [1] original UPC code for each brand of lens purchased
c) Copy of eye exam receipt and/or contact lens fitting receipt with date circled
